What You’ll Do: Deliver exceptional, patient-centered care tailored to individual needs. Conduct thorough assessments and adapt care plans to ensure the best outcomes. Collaborate with physicians, therapists, and other healthcare professionals to provide seamless, holistic care. Maintain accurate documentation and ensure compliance with OASIS assessments. What We’re Looking For: An active RN license in the state where you’ll practice. At least 2 years of nursing experience (home health experience is a plus!). A compassionate, detail-oriented mindset with strong clinical judgment. Excellent communication skills and the ability to work independently while still being a team player.
